i think they can only see who they are in the mirror and everyone else can see them that way too. think its like body cloaking or something.

I dont understand this it just isnt straight body switching because they would just put sam in that kids body. For some reason he sees him self as him but everyone else seems him as the kid.

I figure it's because Sam is himself within that kid's body, therefore he SEES himself. Others only see the physical form of the kid, not knowing Sam is really him, so they only know to see the kid.

Hope that makes a bit more sense lol.

And Dean would only see Sam as Sam because he wouldn't know otherwise...

No he see's the kid too when he looks in the mirror. I think we the viewers see Sam as Sam to avoid confusion that he's Sam. But to himself and everyone else he looks like that kid.
